Assume that SP will submit to ending her starvation diet. Calculate SP's BMR using the Harris-Benedict equation. What activity factor would you initially use? On a separate sheet of paper, outline a refeeding plan for SP and include in your plan: the number of kcals you would start with per day, the rate at which you would advance, and your final kcals per day.

Question 2

If blood was drawn and analyzed, what results would you expect to find? Explain your expectations.

Answer to Question 1

BEE = 655 + 9.6 x 38.56 + 1.7 x 157.5 - 4.7 x 19
= 655 + 370.2 + 267.8 - 89.3
= 1203 kcals Activity factor of 0.8 = 960 kcals
Anorexics may want to get better but probably will still not want to eat a lot so their refeeding program will be a lot slower than a typical refeeding plan for a patient in the hospital recovering from trauma or disease that has been starved.

If SP starts to gain weight and adjusts to moderate activity, her final BEE could be as high as 1.5 times greater to account for weight gain. Your students may determine her BEE to adjust to a different figure; if they can justify their answers, it will be OK. The final analysis will be up to you. 1.5 x 1200 = 1800 kcals. This should be the final goal.

On the separate sheet of paper, you may want your students to outline specific foods or groups of foods to be included on a daily basis, increasing the total amount weekly until a final; goal is attained. This material should be explained in your lecture and the rate of increase and final goal may vary depending on your teaching.

Answer to Question 2

Depressed Hgb, Hct, and MCV due to protein/Fe deficiency. Depressed Ser Alb and TLC due to protein deficiency. Low Bun and serum creatine due to decreased muscle mass.
