Answer to Question 1

C
Feedback:
The current criterion for performing bariatric surgery is a BMI of 40 or greater or a BMI of 35 to 39.9 for clients who have major comorbidities.

Answer to Question 2

Decreased intake: Ordered diet restriction
Nutrient losses: Hemodialysis
Decreased intake: Complaints of nausea
Decreased intake: Exhibiting signs of depression and social isolation
Increased needs: Chronic venous ulcer on lower leg

Rationale: Malnutrition in the client with chronic renal failure can be due to nutrient losses, decreased food intake, or increased nutrient and caloric needs. Diet restrictions for clients with renal failure, depression, social isolation, and nausea are associated with a decreased intake of kilocalories and nutrients. Clients lose nutrients during hemodialysis with the use of dialysate. Clients who are experiencing inflammation or infection have an increased need for nutrients and kilocalories.
