You are caring for a client with chronic pancreatitis. When reviewing the client's chart, you notice that his dietary orders specify that he should receive enteral nutritional supplements that have a polymeric formula.

You question the order because: 1. Polymeric formulas contain nutrients which are partially digested and easy to absorb.
   2. Clients with chronic pancreatitis require formulas with predigested macronutrients.
   3. Modular formulas would better address his full nutritional requirements.
   4. Isotonic formulas can cause diarrhea and should be avoided.

Question 2

A 32-year-old client at the clinic asks the nurse why breastfeeding is so highly recommended. What benefit would be named that would affect this client later in life?

  A) Breastfeeding is associated with a decreased bone density later in life.
  B) Breastfeeding is associated with an increased risk for hip fracture later in life.
  C) Breastfeeding is associated with reducing the risk of postpartum depression.
  D) Breastfeeding is associated with decreased iron stores later in life.

Answer to Question 1

2
Rationale: Polymeric formulas contain macronutrients that are intact and require the presence of digestive enzymes to be fully digested. Clients with chronic pancreatitis may have malabsorption problems and benefit from formulas that contain predigested nutrients. Modular formulas are not designed to fully meet daily nutritional requirements. Isotonic formulas are less likely to cause diarrhea than hypertonic formulas.

Answer to Question 2

C
Feedback:
Breastfeeding offers benefits to the mother. These include conserving iron stores by prolonging amenorrhea, improving bone density and reducing the risk of hip fracture, and reducing the risk of postpartum depression.
