Answer to Question 1

2

Rationale: Increased serum lipid levels, obesity, and toxic levels of some vitamins indicate the client is suffering from overnutrition. Decreasing muscle mass, poor wound healing, functional decline, immunodeficiency, and poor growth in children are all indicators that the client is suffering from protein-related malnutrition. Anemia and poor bone density would more likely be associated with undernutrition but specifically related to diminished intake of iron or calcium and vitamin D.

Answer to Question 2

C

Your skin wrinkles if you stay in the bathtub a long time because the outermost layer of skin (which consists of dead keratin) swells when it absorbs water. It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.
