Many patients who use drug therapies to suppress HIV infection develop abnormalities in body fat and fat metabolism known as lipodystrophy. Patients may lose fat from the face and extremities, accumulate abdominal fat, or both. Also observed are breast enlargement (in both men and women), fat accumulation at the base of the neck (sometimes called a buffalo hump. The changes in body composition are often disfiguring and may cause physical discomfort; moreover, patients often develop hypertriglyceridemi a, elevated low-density lipoprotein (LDL) cholesterol levels, reduced high-density lipoprotein (HDL) cholesterol levels, insulin resistance, and hyperinsulinemia. The specific reasons for the development of lipodystrophy are unknown.
