The social determinants of health model goes beyond the biological and genetic makeup and behaviors at the individual level. The model organizes the complex and interacting factors that determine health and nutritional well-being of the population and provides a framework so these factors can be monitored. The five social determinants, and the factors they encompass, are:
1 . Economic stability: Poverty, employment, food security, and housing stability
2 . Education: High school graduation, enrollment in higher education, and language and literacy.
3 . Health and health care: Access to health care, access to primary care, and health literacy.
4 . Neighborhood and built environment: Access to healthy food, quality of housing, crime and violence, and environmental conditions.
5 . Social and community context: Social cohesion and support systems, civic participation, and perceived discrimination and equity.
