The Nutrition Care Process consists of nutrition assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoring and evaluation.

Services focus on risk assessment of behaviors such as weight status, dietary and alcohol intake, folate and iron status, and vitamin, mineral, and herbal supplement use, as well as on the presence of diseases such as diabetes, hypertension, infections, and genetic traits that may be transmitted to offspring. Psychosocial needs should also be addressed as part of preconceptional care, and referrals made to appropriate services for issues such as eating disorders, abuse, violence, or lack of food or shelter.
