Answer to Question 1

Perceived Exertion. A method used to determine if a vigorous level of exertion has been attained is the perception of an activity feeling somewhat hard to hard as indicated by breathlessness and/or a fatigue level. Generally a physical effort should allow conversation, not singing, but if one is out of breath quickly or the activity must be stopped to catch breath, then the exertion is too much.
Heart Rate. Another common method used to determine level of exertion is to measure heart rate during or immediately after exercise and compare that rate to a target heart rate or a target zone. Your target heart rate is your recommended heart rate for exercising. Some authorities prefer to recommend a range of heart rates to aim for when exercising referred to as a target zone.
