The RD may plan the nutrition intervention through measures such as prioritizing nutrition diagnoses based on problem severity, referring to policies and program standards when planning, basing interventions using the best available evidence; conferring with the patient, family, and members of the interdisciplinary team; determining patient-centered goals and expected outcomes, developing the nutrition prescription, utilizing standardized language for describing interventions, and identifying resources and referrals when needed.
