 Perceived susceptibility to contracting a disease, such as heart disease, diabetes, or cancer
 Perceived severity of the disease state, such as to what degree the disease will impact his/her life, physically, socially, and emotionally
 Perceived benefits and barriers to adopting a lifestyle that would lessen the impact of the disease
 Self-efficacy, or the belief that one can make the necessary change to lessen health risk
