Several policies are helping to reduce medication errors. The bar codes currently used on medications and patient identification bracelets allow health practitioners to verify that the correct medication and dosage are administered: error messages alert personnel if the drug, dose, or timing of administration is inappropriate. In addition, a national education campaign is attempting to eliminate one of the most common but preventable sources of medication errors-the use of ambiguous medical abbreviations. Because terms such as these are easily misread or misinterpreted, they should no longer be used in clinical documentation related to patient care.
