Answer to Question 1

A Braden score is an assessment of a patient's risk of developing pressure ulcers. The Braden score looks at 6 criteria: sensory perception, moisture, activity, mobility, nutrition, and friction and shear.
Mr. Campbell's Braden score is 17 . This indicates that he is currently at mild risk of developing pressure ulcers. Nutrition relates to the Braden score because it is one of the 6 criteria involved. Poor nutrition can lead to poor wound healing, poor skin turgor, decreased mobility/ activity, and decreased sensory perception and increases the risk of the patient developing a pressure ulcer. Poor nutrition also makes it more difficult for pressure ulcers to heal.
