Liability can attach when an individual is not at fault (strict liability). Also, a person or organization may be held liable for the actions of another (vicarious liability).
Avoiding, reducing, or mitigating liability involves a comprehensive plan to prevent incidents that create liability and to take action to limit liability if an incident occurs.
