Answer to Question 1

Answer: 1

1. Stress incontinence involves the loss of small amounts of urine when intra-abdominal pressure increases, as with sneezing or coughing. The pads help to keep undergarments dry.
2. A bedside commode would help with urge incontinence.
3. Catheterization for residual urine would help with functional incontinence.
4. Toileting every 2 hours would help with overflow incontinence.

Answer to Question 2

Answer: 4

1. If present, hypertension should be controlled, but the blood pressure elevation is not severe.
2. A urine output of 3 liters per day is not realistic.
3. The client does not need a 5000-calorie diet.
4. Preservation of renal function is a primary goal; bed rest lowers metabolic demands on the body and conserves energy reserves.
