Who are Providers of Service under Medicare? What are the eligibility requirements? List five or more nonparticipating facilities that Medicare does NOT make payments to.

Answer:
Providers of services and medical equipment suppliers under Medicare must meet all licensing requirements of the state in which they are located. To be a participating provider under the Medicare program, they must meet additional Medicare requirements before payments can be made for their services. All providers must bill Medicare for a patient with Medicare benefits regardless of that provider's participation in the program.
Medicare does not pay for the following care received in nonparticipating facilities:
 Hospital care
 Skilled nursing facility
 Home health agency
 Hospice
 Outpatient rehabilitation
 Dialysis facilities
 Ambulatory surgical centers
 Independent physical therapists
 Independent occupational therapists
 Clinical laboratories
 Portable x-ray suppliers
 Rural health clinics
