 Altered nutrition-related laboratory values related to chronic kidney disease as evidenced by increasing BUN, Cr, PO4.
 Excessive mineral intake related to consumption of high-phosphorous and high-potassium foods as evidenced by serum PO4 level and intake of >1200 mg, intake of 3900 mg of K, excessive consumption of meats, and usual consumption of orange juice, potatoes, and broccoli (high-K foods).
 Predicted suboptimal nutrient intake related to nausea, metallic taste, and edema as evidenced by patient interview.
