The malignant plasma cells in plasma cell myeloma are found in the bone marrow. Occasionally, but not often, they circulate in the peripheral blood. When they exceed 20 in the peripheral blood, or the plasma cell count exceeds 2.0  109/L, it is called plasma cell leukemia.

Chronic lymphocytic leukemia is characterized by the accumulation of small lymphoid cells in the peripheral blood, with a lymphocytosis greater than10  109/L. Smudge cells, representing disintegrated lymphoid cells, are also present. They are of diagnostic significance.
