Incident reports generally include the names and identifying information of any clients and healthcare personnel involved in the incident as well as information on witnesses; the location, time, and date of the incident; and if a medication is involved, the medication's name and dosage. There should be no entry in the medical record that an incident report was completed. The number of hours the nurse worked before the incident occurred is not a part of the report.

The best action to prevent injury is to label the machine with information about the frayed cord and notify engineering or the department responsible for equipment safety. The nurse would obtain another machine, but placing the machine with the frayed wire in the equipment room puts another client at risk.
