Anesthesia for diagnostic arteriography on patient with history of severe coronary artery disease. This patient has severe systemic disease. An anesthesiologist provided anesthesia. Assign the CPT anesthesia code with appropriate CPT physical status and HCPCS level II anesthesiologist modifiers.

Anesthesia for open or surgical arthroscopic procedures of the elbow, repair of nonunion of humerus. This is a patient with mild systemic disease whose pathological humerus fracture has not healed properly due to osteoporosis. An anesthesiologist provided anesthesia. Assign the CPT anesthesia code with appropriate CPT physical status and HCPCS level II anesthesiologist modifiers.
