Anesthesia for diagnostic arteriography on patient with history of severe coronary artery disease. This patient has severe systemic disease. An anesthesiologist provided anesthesia. Assign the CPT anesthesia code with appropriate CPT physical status and HCPCS level II anesthesiologist modifiers.
