Answer to Question 1

C
Burping the infant frequently during feedings is the best intervention, because air is a common factor in regurgitation. Although positioning on the right side after feedings will help prevent reflux, it is not the best answer, because this occurs after the feeding. Also, preterm infants require smaller, more frequent feedings. Giving small amounts of glucose water in between feedings does not provide nutrition, and thin liquids are more associated with regurgitation than thicker liquids.

Though Candida and Aspergillus species are the most common fungal pathogens causing invasive fungal disease in the immunocompromised, infections due to previously uncommon hyaline and dematiaceous filamentous fungi are occurring more often today. Rare fungal infections, once accurately diagnosed, may require surgical debridement, immunotherapy, and newer antifungals used singly or in combination with older antifungals, on a case-by-case basis.
