Answer to Question 1

C
Increased scrotal heat negatively impacts on the sperm count. Sperm counts are based on testicular and hormonal function. Erectile dysfunction will affect fertility, but not the sperm count. Poor nutrition may affect physical activity, but is not directly related to the sperm count.

Answer to Question 2

D
Collecting more data in this situation may afford the nurse the opportunity for clarification of false information that affected the father's decision. However, it remains his decision, and the nurse's responsibility is to respect and support his decision. Telling the father that most fathers are present for the birth and pleased with the experience could encourage guilt feelings in the father. Asking the father why promotes a block to therapeutic communication. Telling him it is his choice would not be the best response.
