Crosby, known for his zero defects concept, defined quality as conformance to requirements. He believed that the system for creating quality is prevention of errors instead of appraisal.

The quality assurance program recommended by JCAH was a retrospective review of patient care through a closed-chart audit that focused on patient care outcomes.
