Answer to Question 1

D
Feedback:
If a client asks for a PRN medication, the nurse should first assess what symptoms have prompted the client's request. Next, consult the MAR to determine when the client last received the PRN or a regularly scheduled pain medication, and if the prescribed period has elapsed, administer the medication as soon as possible. If the prescribed period has not elapsed, the nurse should inform the client when he or she can receive the next dose. If the pain is extreme, the nurse should consult the primary provider to determine if an additional dose or another medication can be given.

Answer to Question 2

D
Feedback:
Oxygen given in high concentration over many days can result in oxygen toxicity, manifested as changes in lung tissue. In some people, increased oxygen concentrations also affect their ventilatory drive control mechanisms, which actually weaken the stimulus to breathe.
