Question 2

A nursing student is caring for an older adult client admitted to the hospital for treatment for a deep venous thrombus (DVT).

The client and husband are both upset because they had to drive almost 100 miles to get the treatment they believed they needed because the hospital emergency department near their home informed them that it was hospital policy to stop diagnostic testing after 5 pm, and since the client was stable, she could come back in the morning. The client's husband asked the nursing student to explain what agency or agencies he needed to contact to help prevent potential harm to others in the future? The nursing student should state: The first step you should take is to report the occurrence to:
  A) the Joint Commission because it is the agency that sets the standards and provides accreditation for the hospital.
  B) your state board of health, which regulates and licenses the hospital and deals with any health facilityrelated violations.
  C) your local ombudsman for seniors who will act as a client advocate for you and others.
  D) the hospital administrator and discuss the situation and ask for a remedy that meets your satisfaction.

Answer to Question 1

Ans: A
Feedback:
Facilities that accept Medicare or Medicaid funds for their services must be accredited by an appropriate accrediting agency such as the JCAHO.

Answer to Question 2

Ans: D
Feedback:
The older adult client and her husband first need to contact the hospital administrator to discuss their concerns and ask for a remedy that meets their satisfaction. All of the other agencies listed require that the client and family discuss the situation first with the hospital administration, and if the situation is not resolved, then contacting the other listed agencies is acceptable. The next best contact would be the ombudsman for seniors because the agency acts as a client advocate and will guide the client through the entire process and will do a lot of the work of contacting other agencies as necessary in order to find the best solution for the issue. An Internet search should reveal toll-free phone numbers available for the client's area ombudsman, State Board of Health, and the Joint Commission report line.
