Answer to Question 1

3. An informal leader on the unit.

Rationale:
The best people to use for mentoring are the recognized informal leaders on the unit. These nurses are usually knowledgeable and respected and can offer the new nurse support. A nurse with only 1 year experience is still learning in this highly challenging setting and is generally not ready to mentor others. A charge nurse can be a role model and a mentor for a nurse interested in stepping into the charge nurse role. However, the demands on the time of the charge nurse make it more difficult for them to be mentors. It would not be appropriate for the manager of the unit to be a mentor for a staff nurse as the manager's job is mainly administrative.

Answer to Question 2

3. Pulmonary embolism

Rationale:
The presence of a deep vein thrombosis is a risk factor for the development of a pulmonary embolism. The nurse should design interventions to help prevent that development. There is less likelihood that the thrombosis would cause myocardial infarction, renal failure, or pneumonia.
