Incomplete combustion from improper installation or poor maintenance.

Answer to Question 2

Usually the oil-air mixture is incorrect because of the blocked combustion chamber, leading to delayed ignition. When the oil does ignite the small explosion produces a puff of soot and unburned oil that forces its way out into the area around the furnace.
