Answer to Question 1

Digoxin: Cardiac glycoside, increases the force and velocity of myocardial contraction. Anorexia, N&V, excessive salivation, epigastric distress, abdominal pain, diarrhea, bowel necrosis.

Synthroid: thyroid medication, a synthetic sodium salt of T4 . N&V, abdominal pain, diarrhea, and appetite changes.

Cordarone: Antiarrhythmic, class III. N&V, abdominal pain, diarrhea, constipation, anorexia, abnormal taste and smell, and abnormal salivation.

Considering just the fact that he has been small all his life and is now 96 of his average UBW is not a bother. However, considering that he has cancer, and knowing that the better nutritional state a person is in the better the chances for survival, it is of some concern. Is his loss in weight due to average conditions or is it due to the cancer?

Answer to Question 2

 Depressed serum albumin and  Lymph indicate protein malnutrition.
 Mg and P indicate improper or inadequate total nutritional intake.
 Hgb, hct, MCV, MCH indicate anemia that could be due to inadequate intake of iron, protein, or food in general.
