Answer to Question 1

Two Cal HN is too high in protein and is very concentrated. Now that he has recovered, his fluid requirement is about 1 cc per kcal. He is receiving half of that and no fiber to help with laxation. His inactivity is causing calcium to come out of the bones. Since he is gaining weight, his Harris-Benedict BEE estimation is of little value; he obviously needs fewer kcals. I would therefore look for an enteral formula that is less concentrated, that has less protein, is not high in calcium, and contains fiber.

Answer to Question 2

No. Garlic can be preserved in many different ways and the method of preservation can have a profound effect on the concentration of ajoene. For example, dried garlic preparations do not contain ajoene. They do contain alliin from which ajoene can be made but it requires the action of the enzyme alliinase. Whether it is present in dried garlic or not depends on the way the garlic was dried. Even if it is there, it is destroyed by a acid medium so the stomach acid inactivates it. If the garlic pill is enteric coated and has been dried properly, it should have some activity1.
