Answer to Question 1

The health continuum depicts health as a continuum from a well population to a population at risk to a population with an established disease to one with a controlled chronic disease. Health promotion is more effective when the programming is targeted to the level of health within the population. Primary prevention activities are best for those who are well whereas tertiary prevention activities are more appropriate for those with an established chronic disease.

Answer to Question 2

Community nutrition is the broader of the two terms and encompasses any nutrition program whose target is the community, regardless of the funding source. Public health nutrition refers to those community-based programs conducted by a government agency whose official mandate is the delivery of health services to the individuals living in a particular area.
