Unclear patient records may cause difficulty for a provider in providing the best quality care. Without all of the pertinent information, a provider may be missing details that affect how the patient may be best cared for. Missing information may also affect the office manager in that he or she may be unable to address a patient's concern clearly about the type or quality of care received, if the chart notes are not complete or clear.
