Chuck is a 63-year-old man who was scheduled to undergo a hiatal hernia repair. He has a history of hyperlipidemia, the aforementioned hiatal hernia, HTN, and GERD. As part of his pre-op clearance, he had an EKG that showed a right bundle branch block. He is here for further work up of this finding. He is scheduled for labs (to include troponin level, creatine kinase, and ESR) along with myocardial perfusion studies and a cardiac catheterization.

Carlin is a 62-year-old woman whose sister, Gail, recently had a cerebral infarction. Fearing what her sister had experienced, Carlin made an appointment for a physical with her family doctor. Carlin's doctor found a carotid bruit on auscultation, and ordered additional tests to rule out carotid stenosis. Doppler ultrasound was highly suspicious for left-sided carotid stenosis. It was confirmed on angio CT. The artery was 85 occluded. The patient is here for an endarterectomy. Patient has a history of thyroid cancer, which was successfully treated nine years ago. She also has hypertension and arthritis of the right elbow.
