A client who is 16 weeks pregnant states, I thought the most dangerous time is the first 3 months, so I shouldn't have to worry from now on about any dangers to the baby. An appropriate response by the nurse would be,
 
  a. There are teratogens that may affect your baby during any stage of development.
  b. We really won't be able to say for sure before you have an ultrasound.

However, problems caused by teratogens can affect the fetus at any time during pregnancy. Telling the mother that they won't know until the ultrasound might frighten the mother and it is not an appropriate nursing response. Telling the mother that she doesn't seemed concerned about her baby's welfare is a judgmental statement. Telling the mother she is past the critical point is a false statement.
