Answer to Question 1

A
Teratogens, including nicotine from smoking, varicella, hepatitis B, HIV, environmental chemicals, cytomegalovirus, and drugs, if used or contracted during the first trimester, have a higher incidence of detrimental effects for the fetus. However, problems caused by teratogens can affect the fetus at any time during pregnancy. Telling the mother that they won't know until the ultrasound might frighten the mother and it is not an appropriate nursing response. Telling the mother that she doesn't seemed concerned about her baby's welfare is a judgmental statement. Telling the mother she is past the critical point is a false statement.
